腸促胰激素對自閉癥孩童療效的雙盲交叉評估

目的:探討腸促胰激素(Secretin) 對自閉癥孩童療效的雙盲交叉研究.

材料及方法:在自閉癥總會宣導下,計有58名經各教學醫院診斷為自閉癥的孩童參加研究.參加孩童依性別、年齡、體重和語言能力加以配對后隨機分成兩組,采雙盲交叉實驗.第一次注射后觀察六星期,再交叉注射,再追蹤六星期.實驗采用日本衛材的Secrepan,成份跟Secretin一樣,采肌肉注射.評估使用異常行為檢核表(ABC),Baker量表,SOS整體量表,和家長每周記錄表.實驗組和對照組間采魏氏曼惠特尼考驗和卡方測試,前測和后測間采魏氏帶符號等級考驗.

結果:家長整體評估顯示:腸促胰激素對實驗組自閉癥狀有改善,80%達到輕度及中度的綜合療效(p < 0.05).實驗組和對照組都顯示自閉癥狀的嚴重波動性,每周惡化和改善的比率約在17%-20%.主改善癥狀為語言談話,較好的睡眠,眼接觸,和人際社交,其有效性持續六到十二周.主要惡化癥狀為情緒躁動不安,睡眠障礙,活動量大,和胃口不良.后者和腸促胰激素的副作用混淆不清.

結論:注射腸促胰激素對部分自閉癥孩童有輕度到中度的短期幫助.但這還不是針對自閉癥病因的根本治療,但它卻能改善自閉癥患者的局部常見癥狀,減輕教養上的阻力.
